Our Care Co-ordination service helps families navigate their way through the complexities of health and social care as well as providing more practical support.

The Donna Louise offers a “one-stop” information, advice and support service for families trying to juggle the challenges of never-ending appointments and the minefield that can be health and social care.

Each family is allocated their own Care Co-ordinator who will be a familiar face to guide them through the care and support available both at The Donna Louise and in the wider community.

 

Debbie Askey, Lead for the Care Co-ordination team, explains,

“Our team is here to help reduce the pressure on families. Keeping track of a child’s appointments with a wide range of professionals can be daunting. We can help a family with practical support, such as liaising with other professionals to get them help when problems arise, or we may support the family by attending meetings with them and giving them the chance to talk things over afterwards. Families may also need help with more practical issues such as how to get hold of equipment, where their child go swimming or where they can find accessible playgrounds. Whatever a family needs, the Care Co-ordination team can help families get the help and support they need.”
